To make a Pre-Authorized Donation on a monthly basis to Faith on Fire please follow these steps:

1. Download and print off the form provided and fill in the information.

2. Specify on the form either Bank Debit or Credit Card monthly donation.

3. Be sure to enclose a blank cheque with VOID written across the check if doing a bank debit.

4. Be sure to fill in all the necessary information.

5. Print and keep a duplicate copy of the form for your records.

6. Mail the form to the address that is located at the top of the form or fax it to 519.742.4030.

7. You may cancel your monthly donation at any time by contacting us by email, phone or by mailing a letter.
